The Hammers robbed the Tractor Boys of a place in the final at Cardiff despite losing 1-0 in the first game at Portman Road.
Matthew Etherington gave the home side the lead in the 50th minute before Christian Dailly sealed victory in the 71st.
Despite three minutes added time at the end Ipswich could not get the vital goal to take them in to extra time.
